The Floresville ISD Athletics Department will host a Physicals Day on Wednesday, May 1. Physicals are sponsored by Floresville Elite Care Clinic.

  • Location: FHS Competition Gym

  • Time: 3 p.m. - 6 p.m.

  • Cost: $15 cash only

All incoming 7th, 9th, and 11th graders for the 2024-25 school year must complete a new physical in order to participate or if a student answers any questions 1-6 on the Medical History Form with a “YES” or has had a major surgery, they must complete a new physical, regardless of grade for the 2024-25 school year.

Required Forms & Rank One Instructions

ALL students interested in athletics regardless of grade level must complete and submit the following forms before participation in any practice or game in the 2024-25 school yea:.

  • Medical History Form

    • Back page of the physical form

    • https://www.fisd.us/o/tigersjags

      • Drop down menu, click on “ALL SPORTS FORMS”

      • Look for “PRE-PHYS FORM” on the list and select “DOWNLOAD” 

    • Must be completed by hand

    • Submitted to either the Athletic Trainer or dropped off at the FHS Office

    • Back side of this form is where the physical is completed 

  • Athlete Information/Travel Sheet

    • Online form completed and submitted via Rank One

    • All fields must be completed

      • If no information is applicable,  please fill with “N/A”

  • UIL Forms Signature Page

    • Online form completed and submitted via Rank One

    • All fields must be completed

      • If no information is applicable,, please fill with “N/A”

All current 6th, 8th & 10th grade students interested in athletics are required to get a physical for the 2024-2025 school year.

Current 7th, 9th, 11th grade students who answer “YES” to any of the first seven questions on the medical history form are also required to get a new physical for the 2024-2025 school year.

RANK One is available to complete online.

  • From the homepage, click on the drop down menu and find “LINKS”.

  • Click the link that says “RANK ONE FORMS”. This will open up a different tab.

  • Read and follow instructions from here.

  • Must have your child’s complete name, as listed in the district files and their full student ID number to complete forms online.

  • If you already have a Parent Account, complete the Parent Portal with the e-mail and Password previously used.
